Based on record review and interviews at 11 a.m., S1 failed to notify the Department and DCFS within 24 hours of the following critical incident: On 9/9/2025, at 8:02 p.m., S1 received notification that O1 observed an unknown staff grab and fling C2, 2-year-old, by the arm at pickup earlier that day. The Specialist was unable to validate the allegation. Corrective Action: Effective 9/30/2025, S1 stated she will review and better familiarize herself and staff with the Critical Incident and Required Notification regulation, to ensure compliance with this regulation.
