Based on record review/interview at 1:30 p.m., S1 failed to notify DCFS Child Welfare within 24 hours of the following incident: On 9/30/2025 at approximately 11:15 a.m., C1, 2-years-old, was left unsupervised on the playground for approximately 5 minutes. A DCFS report was completed prior to Specialist exiting the center. Corrective Action: Effective 10/27/2025, S1 will report all incidents to DCFS to ensure compliance with this regulation.

Based on record review/interview at 1:30 p.m., S4 & S7 left C1, 2-years-old, unsupervised for approximately 5 minutes on 9/30/2025 while in the process of transitioning from the playground to inside the center. Corrective Action: Effective 10/27/2025, S1 will change the center's playground transition procedures to ensure compliance with this regulation.S4 and S7 were retrained on supervision procedures.

1915.A. Based on record review on 8/11/2025 at 3:00 p.m., S1 failed to document daily observations results including an explanation from parent and/or child on 8/4/2025 - 8/8/2025. An explanation failed to be documented 16 times. Corrective Action: Effective 8/8/2025, S1 will have director and director designee to check observations twice daily to ensure compliance with this regulation.

Based on observation at 11:00 a.m., the outdoor play space lacked an enclosure that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The rear gate failed to securely latch when closed. Corrective Action: Effective 7/7/2025, S1 submitted a work order for the lock to be repaired to ensure compliance with this regulation. S1 stated the maintenance department is scheduled to fix the gate on Wednesday, 7/9/2025.

1915.A. Based on record review on 7/7/2025 at 2:15 p.m., S1 failed to document daily observations results including an explanation from parent and/or child on 6/30/2025 - 7/3/2025. Corrective Action: Effective 7/7/2025, S1 will review daily observations every morning before noon and contact parents if needed to ensure compliance with this regulation.

1911.E. Based on record review at 12:18 p.m., the daily electronic reports for C1 and C2 were incomplete on 2/26/2025. The Specialist reviewed the electronic daily infant reports and verified S14 failed to include the liquid intake for C1 and the sleeping pattern for C2.

1917.F. Based on record review at 12:55 p.m., S1 failed to obtain a written authorization signed and dated by the parent to apply topical ointments to C1. The Specialist reviewed the daily infant report from 2/26/2025, and observed a cream was applied to C1 during the diaper change at 11:56 a.m.

1919.J. Based on interviews/record review/observation at 9 a.m., S14 failed to have bottle formula for infants labeled with the child's name. The Specialist observed 1 of 14 bottles failed to be labeled. This issue was corrected prior to the Specialist to exiting the classroom. On 2/26/2025, S14 fed C1 a bottle of formula belonging to C2.

1103.B. Based on record review/interview, S1 failed to notify the Department of the following critical incident: On 11/18/2024, O1 notified S1 that C1, 6 month old, was fed a bottle belonging to another child.
