A DCFS report was completed prior to Specialist exiting the center. Corrective Action: Effective 10/27/2025, S1 will report all incidents to DCFS to ensure compliance with this regulation.

The rear gate failed to securely latch when closed. Corrective Action: Effective 7/7/2025, S1 submitted a work order for the lock to be repaired to ensure compliance with this regulation. S1 stated the maintenance department is scheduled to fix the gate on Wednesday, 7/9/2025.
