      1711.A.D.E. Based on observation at approximately 10:30 a.m., the required child to staff ratio failed to be met for children of the following ages: 9 children, ages 2-months-old through 2-years-old, with 1 staff, S4. At approximately 10:45 a.m., 13 children, ages 2-years-old through 3-years-old, with 1 staff, S5. Child to staff ratio was corrected during the inspection. Corrective Action: Effective 6/4/2026, S2 will hire more staff and also assign staff to classrooms to meet ratio requirement to ensure compliance with this regulation.

      Based on observation/record review at 10:30 a.m., S1 failed to ensure staff document observations, noted on children upon arrival to the center, and include an explanation from parent and/or child. The Specialist observed a red bump on C2's, 2-years-old, right cheek. S5 failed to document the bump and failed to include an explanation from the parent. Corrective Action: Effective 6/4/2026, S2 will assign a staff to complete daily observations every morning to ensure compliance with this regulation.
