      1723.A.&B.: Based on observation/record review at 11 a.m., S1 failed to have documentation that 1 of 9 staff, on the premises and accessible to the children had current certification in CPR and first aid certifications through training approved by the Department. S2 CPR certification expired 3/30/2026. No class is scheduled for S2. Corrective Action: Effective 4/30/2026, S1 stated she will create a form to show the expiration of staff trainings to ensure compliance with this regulation.

      Based on record review at 10:30 a.m., S1 failed to ensure the children's daily attendance record included the first and last name of the child, the departure time of each child and the name of whom the child was released to on 1/13/2026-1/14/2026, 2/09/2026-2/13/2026, 3/03/2026-3/06/2026, 4/07/2026-4/08/2026, and 4/23/2026. Corrective Action: Effective 4/30/2026, S1 stated she will remove the children's daily attendance sheet from each classroom and will sign in all children on one form when entering the center to ensure compliance with this regulation.

      Based on record review at 11:45, S1 failed to have at least two staff members trained in medication administration whether the early learning center administers medication or not. S1's medication administration expired on 3/30/2026. S5 is the only staff trained in medication administration. Corrective Action: Effective 4/30/2026, S1 stated she will have staff complete medication administration training on EC Learn to ensure compliance with this regulation.

Corrective Action: Effective 10/14/2025, S1 stated she will have staff meeting on 10/15/2025 and remind staff to provide an explanation to ensure compliance with this regulation.

Corrective Action: Effective 10/14/2025, S1 stated she will assign a specific staff person to complete the tornado drills during the months of March, April, Ma, and June to ensure compliance with this regulation.

      C.: Based on record review at 12:15 p.m., S1 failed to provide documentation for S8 (DOH: 1/06/2025) LDE Key Training Module 1 was completed within 7 days and the LDE Key Training Modules 2-3 were completed within 30 days of first day present at the center on 1/06/2025. S1 stated S8 completed the trainings but did not print them off of desktop computer away from the center.
