      Based on observation at 1:15 p.m., S1 failed to have a functional, readily available telephone capable of incoming and outgoing calls at all times at the center. Corrective Action: Effective 1/05/2026, S1 stated she will contact the telephone company to request repairs to the telephone line to ensure compliance with this regulation.

      Based on record review at 12:00 p.m., S1 failed to have a Child's Information Form for C1 and C2, age 3 years old, of the cumulative files reviewed by the Specialist. This was not corrected prior to the Specialist departure. Corrective Action: Effective 1/09/2025, S5 stated she will remind S1 to obtain Child information form from parents to ensure compliance with this regulation.

      1719AC Based on record review at 1120 am S1 failed to have documentation that S8 completed LDE Key Training Module 1 within 7 days of the first day present at the center; and LDE Key Training Module 2, 3 within 30 days of the first day present at the center. S8s hire date was 11/11/2024. S8's first day working was 11/12/2024. This was not corrected prior to the Specialist departure. Corrective Action: Effective 1/09/2025, S5 stated she will remind S8 to complete LDOE module trainings to ensure compliance with this regulation.

      Based on interview(s) at 2:30 p.m., although the center established a behavior management policy, S1 failed to ensure the behavior management policy was followed on unknown dates/times when S7 (DOH 5/09/2022) utilized a harsh voice with C1, 3 years old, by being a little frustrated. S7 remains employed at the center.

Based on record review/interview at 10:00 a.m., although the center established a behavior management policy, S1 failed to ensure the behavior management policy was followed on unknown dates/times when S7 (DOH: 5/09/2022), utilized a prohibited method of time on C1, 3 years old, by placing C1 in a high chair for timeout. Timeout was also exceeded over the age of the C1. S7 remains employed with the center.

      1915.B.C. Based on observations/record review at 12:30 p.m., S1 failed to have documentation of incidents, injuries, accidents, illnesses, and unusual behaviors demonstrated by C1, 3 years old. S1 stated C1 hit and scratched other children on an almost daily basis.
