C.: Critical Incidents and Required Notifications: Based on record review at 9:30 a.m., S1 failed to notify the Department within 24 hours of the following incidents: on 10/9/2024, at 3:10 p.m., O1, C1's mother, notified the center of S3 allegedly making the class laugh at C1, 4-years-old, when she fell out of the chair. On 10/11/2024, after further investigation, C1 also reported to S2 that S3 pinched her. S1 submitted a CIR to the Department on 10/21/2024.

: Based on interview(s) at 11 a.m., although the center has a behavior management policy in place that prohibits physical/corporal punishment, S3 used a prohibited method of discipline, corporal punishment. S3 made the class laugh at C1, 4-years-old, when she fell out the chair. S3 aggressively put C1 in timeout when she misbehaved and also pinched her. S3's date of hire is 8/20/2024; her termination date is 10/11/2024.
