The Specialists observed C1, age 2-years-old, leaving out of the classroom and going into another classroom, alone and unsupervised, while S4 was supervising the other children. S4 retrieved C1 at 10:40 a.m. Corrective Action: Effective 03/02/2026, S1 stated that he will retrain all staff on supervision and conduct daily walkthroughs throughout the day to ensure compliance with this regulation

The Specialist observed S5 supervising 5 children, ages four-years-old, alone in the classroom. This was corrected at 11:09 p.m. when S5 and S4 combined classrooms. Corrective Action: Effective 10/13/2025, S1 stated she will have new staff paired with a staff that has current training until all required training is completed to ensure compliance with this regulation.
