Based on observation and interviews at 10:35 a.m., S4 failed to ensure all children were under supervision at all times. The Specialists observed C1, age 2-years-old, leaving out of the classroom and going into another classroom, alone and unsupervised, while S4 was supervising the other children. S4 retrieved C1 at 10:40 a.m. Corrective Action: Effective 03/02/2026, S1 stated that he will retrain all staff on supervision and conduct daily walkthroughs throughout the day to ensure compliance with this regulation

1723.F. Based on record review at 11:01 a.m., S1 failed to ensure that S5 (date of rehire 9/2/2025) had current certification in pediatric first aid and CPR before having sole responsibility of the children. The Specialist observed S5 supervising 5 children, ages four-years-old, alone in the classroom. This was corrected at 11:09 p.m. when S5 and S4 combined classrooms. Corrective Action: Effective 10/13/2025, S1 stated she will have new staff paired with a staff that has current training until all required training is completed to ensure compliance with this regulation.

1811.A.B. Based on record review/interview at 10:53 a.m., S1 failed to have documentation that a CCCBC-based determination of eligibility (CCCBC) was conducted for S5 (date of rehired 9/2/2025) prior to S5 being present in the childcare facility. S5s CCCBC-based determination is expired on 2/11/2024. S5 was present at the center on 9/2/2025-9/5/2025, 9/8/2025-9/12/2025, 9/15/2025-9/18/2025, 9/22/2025-9/26/2025, 9/29/2025-10/3/2025, 10/6/2025-10/10/2025, and 10/13/2025. 1901.C. Based on record review at 1:06 p.m., S1 failed to have documentation that the entire center and play yard was checked after the last child departed to ensure that no child was left unattended at the center for the following days: 8/7/2025, 8/8/2025, 8/11/2025, 8/12/2025, and 8/13/2025. Corrective Action: Effective 8/14/2025, S5 stated she will review the visual check binder every evening for documentation to ensure compliance of this regulation.

Based on record review at 12:56 p.m., S1 failed to maintain documentation of a daily attendance record for Staff and Owners, to include the time of arrival and departure for the following days:-On 8/12/2025, 5 of 5 staff had no documentation of departure times.-On 8/13/2025, 5 of 5 staff had no documentation of arrival and departure times. -On 8/14/2025, 4 of 5 staff had no documentation of arrival times. Corrective Action: Effective 8/14/2025, S5 stated she will review the attendance log every morning and evening for documentation to ensure compliance of this regulation.
