Based on record review/interview at 10:30 a.m., on 02/12/2025, S1 failed to immediately notify the parent, and failed to notify the Department and Child Welfare within 24 hours of the following critical incident: On 01/31/2025, at approximately 10 a.m., S3 stated she witnessed S13 dragging a child down the hallway while S1 walked behind them. S3 notified S12 of the incident on 02/03/2025, and S12 notified the Department and Child Welfare on 02/04/2025. S1 contacted O1 to inform her of the incident on 02/05/2025. Specialist could not validate the incident.
