      Based on record review and interviews at 12:53 p.m. on 05/12/2026, although the center has a behavioral management policy implemented, S17 (DOH 09/27/2024) was observed by a staff member and on center’s camera using a prohibited method of discipline as C1 and C2, both age, 5-years-old were subject to physical punishment. On 05/06/2026, S17 yanked C1 by the hair and yelled at him because he got out of line, and hit C2 on the head with a clipboard and kicked her backside with her foot. S17 was terminated on 05/06/2026.
