      Based on observation at 9:55 a.m., S1 failed to meet the required child to staff ratio for children of the following ages: 18 children, four-to-five-years-old, with S7. The required ratio for children of this age is 17 children per 1 staff person. Child to staff ratio wasn't corrected during the inspection. Corrective Action: Effective 05/06/2026, S1 stated she will hire additional staff, to ensure compliance with this regulation.

      Based on observation at 9:05 a.m., S1 failed to meet the required child to staff ratio for children of the following ages: 15 children, three-to-four-years-old, with S2. The required ratio for children of this age is 14 children per 1 staff person. Child to staff ratio was corrected during the inspection. Corrective Action: Effective 02/25/2026, S1 stated she will retrain all staff on child to ratio and instruct staff to notify her when the ratio is met at drop off, to ensure compliance with this regulation.

      Based on observation at 9:30 a.m., S1 failed to ensure the outdoor play space was enclosed in a manner that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons as the fence gate does not securely latch when closed. This was not corrected during the inspection. Corrective Action: Effective 02/25/2026, S1 stated she will have the gate lock cut and notify contractors to refrain from placing unauthorized locks on outdoor gates, to ensure compliance with this regulation.
