Based on observation at 12:20 p.m., S4 failed to ensure the outdoor play space was enclosed with a permanent fence or other permanent barrier in a manner that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The specialist observed the outdoor play space without a permanent fence. Corrective Action: Effective 4/15/2025, S4 stated she will have all fence work completed when children are not on premises to ensure compliance with this regulation.
