      Based on record review on 9/24/2025 at 11:00 a.m., S1 failed to notify the Department within 24 hours of the following incident: on 8/18/205, at 3:17 p.m., S10, asst teacher, stated she felt S8 and S9 were talking mean to the children in a loud and harsh tone. Child Welfare, Law enforcement nor emergency personnel were notified. S1 submitted a CIR to the Department on 8/21/2025, after being informed by management that she needed to do so.
