      1103-A.5C.1.2. Based on interviews at 12:30 p.m., S1 failed to notify the Department and DCFS within 24 hours of the following critical incident: On an unknown date in August of 2025, at an unknown time, S1 was notified by O1 that C1, age two-years-old, stated S7 popped her on top of her head and told C1 No! Don't do that. Corrective Action: Effective 10/09/2025, S1 stated she will submit all critical incidents to the department and DCFS upon notification to ensure compliance to this regulation.

      1711.A.B.D.E. Based on observations at 9:37 a.m., S1 failed to meet the required child to staff ratio for 1-year-old children. The Specialists observed S4 caring for 9 children, ages 18-months to 2-years-old, alone. The required ratio for one-year-old children is 7 children per 1 staff person. One additional staff was needed to meet ratio. Ratio was met at 9:45 a.m. when S2 went into the classroom with S4. Corrective Action: Effective 10/09/2025, S1 stated she will ensure there are enough staff members in each classroom to ensure compliance with this regulation.

      1915.B.C. Based on record review at 12:00 p.m., S2 failed to have documentation of notification to the parent when the following occurred: S10 observed several scratches on C1s, age three-years-old, neck and placed a bandaid on C1s neck. The incident occurred on 5/7/2025 at 4:00 p.m. There was no documentation of parental notification. Corrective Action: Effective 05/22/2025 S2 stated she will check parental notifications and documentation of all incidents and accidents at the center at the end of each day to ensure compliance with this regulation.
