Specialist observed the following: S6, Date of Hire: 7/14/2025, completed Mandated Reporter Training on 7/20/2025. Corrective Action: Effective 8/18/2025, S1 will have all new staff to complete orientation training prior to their first day of work to ensure compliance with this regulation.

. Based on observation at 12 p.m., S1 failed to ensure that children under age 4 do not eat foods that are implicated in choking incidents. Specialist observed 10 children, age 2-years-old, eating whole chicken nuggets in S2's and S3's classroom. This issue was corrected prior to Specialist exiting the classroom. Corrective Action: Effective 8/18/2025, S1 will chunks of meat into sizes that can be swallowed whole prior to serving food to ensure compliance with this regulation.
