The center's commercial liability insurance expired on 5/6/2026. Corrective Action: Effective 5/22/2026, S2 will remind S1 renew a month prior to the expiration date to ensure compliance with this regulation.

      Based on record review at 10:15 a.m., S1 failed to have documentation that 1 of 10 staff, S7, completed The DCFS online Mandated Reporter Training within seven days of the first day present at the center on 12/2/2026. Corrective Action: Effective 5/22/2026, S2 will require all new hires to complete training prior to their first day working to ensure compliance with this regulation.

      Based on record review at 11:30 a.m., S1 failed to have documentation that 1 of 10 staff completed DCFS online Mandated Reporter Training within seven days of the first day present at the center. Specialist observed the following: S6, Date of Hire: 7/14/2025, completed Mandated Reporter Training on 7/20/2025. Corrective Action: Effective 8/18/2025, S1 will have all new staff to complete orientation training prior to their first day of work to ensure compliance with this regulation.

    • Food Service and Nutrition - Choking Hazards1919.D.2

      . Based on observation at 12 p.m., S1 failed to ensure that children under age 4 do not eat foods that are implicated in choking incidents. Specialist observed 10 children, age 2-years-old, eating whole chicken nuggets in S2's and S3's classroom. This issue was corrected prior to Specialist exiting the classroom. Corrective Action: Effective 8/18/2025, S1 will chunks of meat into sizes that can be swallowed whole prior to serving food to ensure compliance with this regulation.
