1711.A.B.D.E Based on observations/interviews at 11:43 a.m., S1 failed to ensure the required child to staff ratio was met for one year olds. The Specialists observed S3, alone, supervising 11 children, age 1-year-old. The required ratio for this age group is 7 children per 1 staff person. One additional staff was needed to satisfy ratio. Ratio was corrected at 12:06 p.m. when S2 removed 4 children out S4’s classroom and into S6 and S7 classroom. The Specialists observed S5, alone, supervising 15 children, age 3-years-old. 1713.B. Based on observations at 12:09 p.m., S8 failed to ensure that children were supervised at all times. The Specialists observed O1, Therapist, alone, with 5 children, age 7 to 10 months old. S8 returned to the classroom at 12:10 p.m. Corrective Action: Effective 02/10/2026, S2 stated she will remind staff outside providers are not counted in ratio and staff should call if they have to leave the classroom to ensure compliance with this regulation.

1711.D. Based on observations/interviews at 9:50 a.m., the Specialist observed S3 caring for 6 children, ages 4- months- to 1-year-old in the infant classroom. The required ratio for infants to one year old children is 5 children per 1 staff. One additional staff was needed to satisfy ratio. Ratio for infants was corrected at 10:10 a.m. when S4 went into the infant classroom. Corrective Action: Effective 12/1/2025, S1 stated she will do random walk-throughs throughout the day to ensure compliance with this regulation.

1719.B. Based on record review at 11:30 a.m., S1 failed to have documentation that S3(DOH 05/19/2025) completed the Online Child Abuse and Neglect Mandated Reporter Training provided by DCFS within seven days of the first day present at the center. S3’s first day working at the center was 05/19/2025. S3 completed the Online Child Abuse and Neglect Mandated reporter Training on 10/30/2025. Corrective Action: Effective 12/1/2025, S1 stated she will have newly hired staff complete required training within the first 5 days of hire to ensure compliance with this regulation.

1723.A.B. Based on record review at 1:00 p.m., S1 failed to have documentation that 1 of 9 staff on the premises and accessible to the children had current certification in Infant and Child Pediatric First Aid through a trainer approved by the department. S6 did not have documentation of a current approved certification. Corrective Action: Effective 12/1/2025, S1 stated she will have staff complete training within 60 days before expiration to ensure compliance with this regulation.

1723.C. Based on record review at 1:00 p.m., S1 failed to have documentation that 1 of 9 staff on premises and accessible to children had current certification in Pediatric First Aid through a trainer approved by the department. S6 did not have documentation of a current approved certification. Corrective Action: Effective 12/1/2025, S1 stated she will have staff complete training within 60 days before expiration to ensure compliance with this regulation.

1723. F. Based on record review at 11:45 a.m., S1 failed to have documentation that S3 had current certification in Infant and Child Pediatric First Aid through a trainer approved by the department within 90 days from date of hire. S3’s (DOH 05/19/2025) certification was completed on 11/05/2025. Corrective Action: Effective 12/1/2025, S1 stated she will have staff complete training within 90 days to ensure compliance with this regulation.

1727. A. & B. Based on record review at 12:00 p.m., S1 failed to have documentation that S7 and S8 completed the required annual Online Child Abuse and Neglect Mandated Reporter Training provided by DCFS. S7’s mandated reporter certificate expired on November 08, 2025. S8’s mandated reporter certificate expired on November 07, 2025. Corrective Action: Effective 12/1/2025, S1 stated she will set a reminder 30 days prior to certificate expiration to ensure compliance with this regulation.

1901.Q. Based on observations at 10:50 a.m., S1 failed to have the current ' Safety Box' newsletter issued by the Louisiana Office of the Attorney General posted in the center. Specialists observed the posted safety box from to July 2025. Corrective Action: Effective 12/1/25 S1 stated she will post the safety box every three months to ensure compliance with the regulation.

1507.A. Based on record review at 10:15 a.m., S1 completed the Children's daily attendance record and the record did not include the first and last name to whom child was released on the following dates:-On 4/1/2025, daily attendance log failed to have whom the child was released to for 5 of 44 children.-On 4/2/2025, daily attendance log failed to have whom the child was released to for 10 of 47 children. Corrective Action: Effective 5/21/2025, S1 stated she will list whom child is released to on all daily attendance records to ensure compliance with this regulation.

1719.A.Based on record review at 10:30 a.m., S1 failed to have documentation that S9 (DOH 3/28/2025) completed the DCFS online Mandated Reporter Training within seven days of the first day present at the center. Corrective Action: Effective 5/21/2025, S1 stated she will have all newly hired staff complete the required training with the first 5 days present at the center.
