Based on record review/interviews at 1:38 p.m., S1 failed to notify the Parent immediately of the following critical incident: On 10/24/2024, at approximately 3:45 p.m., C2, age two-years-old, was left unsupervised on the outdoor play yard for approximately five minutes. S30 found C2 outside, crying, at 3:50 p.m. The parent was notified at 4:30 p.m., after C2 was picked up at 3:50 p.m. LDOE was notified on 10/24/2024. Child Welfare was notified on 10/24/2024.
