Based on interviews/observation viewed on 3/12/2025, at 4:30 p.m., S12 failed to ensure children were supervised at all times as on 3/10/2025, at 3:10 p.m., C1, age three-years-old, walked out of his classroom, out of the front door of the center, and into Gauze Blvd. O2 and an unknown passerby retrieved C1 from the street and O2 brought C1 back into the center at 3:11 p.m. S12 was unaware that C1 was not in the center with the rest of the class. S12 was terminated on 3/12/2025.
