C.: Based on record review and interviews at 2:45 p.m., S1 failed to notify LDOE and DCFS within 24 hours and the parent immediately of the following critical incident: On 2/13/2025, at 10:55 a.m., S15 witnessed S22 spank C1, age 3-years-old, on the bottom in the restroom. C1 reported the incident to O1 on the evening of 2/13/2025, after pick-up. O1 reported the incident to S1 on 2/14/2025, at approximately 8:00 a.m. S1 notified DCFS on 2/14/2025, at 4:20 p.m. and LDOE on 2/14/2025, at 4:50 p.m.
