Based on record review/interviews at 12:15 p.m., on 2/19/2026, S1 failed to notify LDOE and DCFS within 24 hours of the following critical incident: In October 2025, a parent notified S1 that C1, age four-years-old, told her S2 touched her on her vaginal area. The Supervisor was not able to validate the allegation. Corrective Action: Effective 2/19/2026, S1 stated she will notify LDOE and DCFS of all allegations within 24 hours to ensure compliance with this regulation.
