1713.A.B Based on interviews at 3:27 p.m., S9 and S10 failed to ensure children were under supervision at all times. On 5/14/2026 at approximately 2:51 p.m., C1, age 1 year-old, left the outside patio area and went into her classroom, alone and unsupervised, for approximately 1 minute as C1’s class transitioned from the outside patio area to the classroom. C1 was retrieved by S8 at 2:52 p.m. Corrective Action:

Based on record review and interviews at 3:54 p.m., staff failed to ensure children were not placed in a bouncer for more than 30 consecutive minutes. S6 stated on 01/23/2024 at 11:30 a.m., C3, age 2-years-old was placed in a bouncer for approximately 1 hour. S5 retrieved C3 from the bouncer when she returned from lunch at 12:30 p.m. Corrective Action: Effective 05/28/2026, S2 stated that she will place timers in the classroom to ensure compliance with this regulation.

Based on record review and interview at 12:00 p.m., S1 failed to notify the parent immediately and LDOE and Child Welfare within 24 hours following critical incident: On 3/03/2026 at 9:50 a.m., S3 hit C1, age 1-years-old, with a closed fist on the left side of her face, as she attempted to bite another 1-year-old child, on the right back upper shoulder. S1 notified the parent on 3/5/2026 at 12:00 p.m. and submitted a critical incident report to LDOE and a written report to Child Welfare on 3/5/2026. 1509.A.8.a.b Based on record review and interviews at 12:00 p.m., although the center has a behavioral management policy implemented, S3 (DOH 8/25/2025) used a prohibited method of discipline as C1, age 1-years-old, was subject to physical punishment. On 3/03/2026 at 9:50 a.m., S3 was observed hitting C1 on the left side of her face with the back of her right hand, for biting another 1-year-old child, on the right back upper shoulder. S3 was terminated on 3/03/2026. Corrective Action: Effective 3/16/2026, S1 stated she will retrain staff on the policy to ensure compliance with this regulation.

Based on record review on 1/27/2026 at 12:30 p.m., S1 failed to maintain documentation of a daily attendance record for visitors to include arrival and departure times, and purpose of the visit for O2 and O3. Corrective Action: 01/27/2026, S1 stated she will check the daily attendance daily to ensure visitors are signing in and out to ensure compliance with this regulation.

1911.I.&J.: Based on interviews at 12:30 p.m., S3 failed to lift C1, age 11-months-old, using proper lifting techniques. On 12/4/2025, at approximately 5:00 p.m., S3 picked C1 up by one arm and moved her away from the gate. Corrective Action: Corrective action plan: Effective 12/16/2025, S1 stated all staff has received training on proper lifting of a child to ensure compliance with this regulation.

Based on interviews and record review on 11/7/2025 at 11:00 a.m., although the center has a written behavior management policy implemented, S3 (DOH 10/18/2024) used a prohibited method of discipline as a child was subject to physical punishment. On 10/24/2025, at 3:20 p.m., S3 pulled C1, age 11-months-old, towards her by one arm then pushed him and told him "go" as he cried while sitting on the rug. Based on interviews on 11/7/2025 at 11:00 a.m., S3 failed to ensure children were under supervision at all times. On 10/24/2025, at approximately 3:20 p.m., C4 was in a bouncer facing away from S3 as she sat in a rocker and on the floor caring for C1, age 11-months - old, and C2, age 6 - months-old. Corrective Action: Effective 11/20/2025, S1 stated she will monitor all classrooms frequently retrain all staff of the Supervision Policy to ensure compliance with this regulation.

1907.A.1. Based on record review and interviews on 11/20/2025 at 10:00 a.m., S7 failed to ensure the manufacturer's restraint devices were used when occupied by children. On 11/11/2025 at 2:05 p.m., S7 unbuckled the restraints of the bye bye buggy that secured C1, age 1-year-old and C6, age one-year-old to put on C1's and C6's jackets while sitting in the buggy. S7 failed to rebuckle the restraints resulting in C1 falling from the buggy, bumping is face on the floor and C6 falling from the buggy, hitting his upper lip, causing the lip to bleed. Based on record review on 11/20/2025 at 10:00 a.m., S1 failed to have documentation of immediate notification to the parent when the following occurred to a child: On 11/11/2025 at 2:05 p.m., C6, age one-year-old, fell from the bye bye buggy and hit his upper lip, causing the lip to bleed. The parent was notified at 2:45 p.m. Corrective Action: Effective 11/20/2025, S1 stated she will immediately notify all parents of any head injuries to ensure compliance with this regulation.

Based on record review and interviews on 9/4/2025, at 11:45 a.m., although a mandated reporter, S22 (DOH 10/5/2023) failed to report suspected abuse of a child to the Child Protection Statewide Hotline. S22 observed S21 slap C1, 1-year-old, in the face on 8/26/2025 at 10:29 a.m. Corrective Action:
