EBRP Wonderland Head Start Center

Based on observations and interviews at 12:00 p.m., on 3/03/2026, S1 failed to ensure a blanket for covering the children on their mats during naptime was provided by the center or the parent. There were 20 students present who did not have a blanket to cover them at naptime. S1 stated the parents did not return the blankets to the center, and they did not have extra blankets. Corrective Action: Effective 3/03/2026, S1 stated she will speak with administrative support to order additional blankets to cover the children to ensure compliance with this regulation.

Based on video footage review, on 11/04/2025, at 11:04 a.m., C2, three-year-old, stepped out of line and leaned on a table. S12 goes over to C2, pulls her by the arm back in line, turns her to face forward, and yanks C2's face upwards with both of her hands on her cheeks. .: Based on observations, record review, and interviews on 5/1/2025, the center failed to have an individual present whose job duties consist solely of administrative duties and of ensuring that staff members working with children do not leave their classrooms to handle administrative duties when the number of children present at an early learning center exceeds 42 children. Corrective Action: Effective 5/1/2025, S2 stated a change request will be made to list S4 as the director designee and/or staff in charge to ensure compliance with this regulation.

Based on record review and interview at 1:00 p.m, S1 failed to maintain documentation of a daily attendance record for all Staff, to include their time of arrival and departure. S2, Staff in Charge, did not have access to the attendance logs to show which staff were in attendance at the center.

Based on record review and interviews at 1:00 p.m. on 12/12/2024, although the center has a Behavior Management policy implemented, on 12/04/2024 at approximately 1:07 p.m. S6 (DOH 08/01/2015) used a prohibited method of discipline on C3, 3-years-old. While C3 was laying on his mat for nap, S6 was seen on video footage yanking C3's blanket off of him and pointing her finger in C3's face stating, You will get it when you go to sleep. Based on record review at 1:00 p.m. on 12/12/2024., S1 failed to ensure all staff devoted their full attention and time to actively supervising all children in the centers care. On 12/04/2024 at approximately 12:57 p.m., while children were napping, S6, S12, and S13 was observed via video footage on their phones while children were napping.

Restrooms Supervision: - Based on record review at 1:00 p.m. on 12/12/2024. S1, Director, failed to ensure that all staff remain in proximity to children while in the restroom. On 12/04/2024 at approximately 12:24 p.m., the Specialist observed (2) three-year-old children in the restroom alone while S13 (DOH 09/16/2014) stood across the classroom at the door guiding other children in the classroom.

Based on record review and interviews at 1:00 p.m., S1 failed to provide documentation that 1 of 13 staff members have an application/staff information form to include their name, date of birth, home address and phone number, training, work experience, educational background, hire date, first day onsite working with children, upon termination or resignation of employment, the last date of employment, reason for leaving, for staff. S2 could not provide a staff file for S6 (DOH 08/01/2015).

Based on record review and interview at 1:00 p.m., S1 failed to ensure that all staff employed at the center had a state or federal government issued photo identification available for review. 2 of 13 staff members, S4 (DOH Unknown) and S6 (DOH 08/01/2025) did not have a current photo I.D. on file.

C. Orientation Training - Based on record review and interviews at 1:00 p.m., S1 failed to provide documentation that 4 of 13 staff, received orientation within seven days of the first day present at the center and prior to having sole responsibility for any children. S1 also lacked documentation that 4 of 13 staff received additional orientation within the first thirty days of date of hire. S1 did not have documentation of training for S3 (DOH 09/28/2023), S6 (DOH 08/01/2015), S12 (08/19/2024), and S13 (DOH 09/16/2014). S2 stated each staff member will have proof of orientation…
