Based on record review/interviews at 1:30 p.m., S1 failed to notify the Department and DCFS within 24 hours of the following critical incident: On 4/16/2025, S3 (DOH 4/29/24) received a text message from S18 (DOH 4/29/24) informing her that S19 (DOH 4/30/24) had jerked C1 , one-year-old. S3 reviewed the cameras and concluded that the incident was not reportable to DCFS or the Department. S3 stated she informed S1 about the incident and asked her to review the footage. Based on record review at 11:30 a.m. S1 failed to ensure the electronic sign in and out procedure for the daily attendance log for children included the time of departure of each child and the name of the person to whom the child was released. On 4/16/25, 3 of 91 failed to have time of departure and first and last name of whom the child was released. Corrective Action: Effective 5/1/2025, S1 stated she will verbally remind parents every day and review the sign out sheet prior to the child leaving the center for the day to ensure compliance with this regulation.
