Based on Record Review at 9:45 a.m., S1 failed to maintain a daily attendance record for children which accurately reflect children on the center premises at any given time and be used to sign in and out if a child leaves and returns to the center during the day. The Specialist observed 7 out of 23 children, not signed into the center for 2/23/2026. Corrective Action: Effective 2/23/2026, S1 stated she will manually have parents sign children into and out of the center, to ensure compliance with this regulation.

Based on Record Review at 10:00 a.m., S1 failed to maintain documentation of a daily attendance record for Staff and Owners, to include the time of arrival and departure. The Specialist observed the staff attendance log for February 23, 2026, but there were no attendance logs for February 1-6, 9-13, or 18-20 of 2026. Corrective Action: Effective 2/23/2026, S1 stated she will manually have staff sign into and out of the center and keep records available at the center, to ensure compliance with this regulation.

Based on observations at 9:00 a.m., S4 failed to follow the Electronic Devices Policy. The Specialist observed television programming displayed in a classroom with 5 one to two-year-old children. Corrective Action: Effective 2/23/2026, S1 stated she will cover televisions in classrooms where children under 2 are present, to ensure compliance with this regulation.

1727.A. Based on record review at 9:30 a.m., S1 failed to provide documentation that all staff completed the online Child Abuse and Neglect Mandated Reporter Training provided by DCFS. The Specialist observed 2 out of 7 staff members did not have Mandated Reporter training. S1 (Expired: 1/14/2026) and S6 (Expired: 1/17/2026), did not have Mandated Reporter training. Corrective Action: Effective 2/23/2026, S1 stated she will require the center's Assistant to review books daily for any expired trainings, to ensure compliance with this regulation.
