      1509.A.12. a-d.: Based on record review and interview at 1:29 p.m., S1 failed to have a written policy describing the monitoring procedure that shall be used at the center when staff members are employed on a provisional basis due to an incomplete CCCBC-based determination of eligibility for child care purposes or any other provisions of the bulletin. Corrective Action: Effective 05/27/2026, S1 stated she will create a policy and posted and provide a copy of the written policy to each parent, to ensure compliance with this regulation.

      Based on observation and interview at 12: p.m., S1 failed to have a log, either handwritten or in electronic form, or other written documentation of the monitoring of a provisionally-employed staff member. S7 (DOH 5/11/2026) has a provisional status background dated 6/22/2026, but S1 did not have documentation for the staff member who monitored her from 5/11/2026 through 5/27/2026. Corrective Action: Effective 5/27/2026, S1 stated she will have all employees with a provisional status have documentation of monitoring to ensure compliance with this regulation.

      Based on record review and interview at 12:00 p.m., the center failed to have a qualified Director/Director Designee, who is an on-site full-time staff person at the center during the daytime hours of operation (before 9:00 p.m.) that is responsible for planning, managing, and controlling the center's daily activities, as well as responding to parental concerns and ensuring that minimum licensing requirements are met. S2 was unable to show proof of S1 being on-site. The Specialist observed the last date the Director was on site was 12/17/2024.
