Based on the record review at 11:33 a.m., S1 failed to have documentation on file for Independent Contractors that included the person's name, address, phone number, and a list of duties performed while at the center. The Specialist did not observe file documentation for O1, O2, O3, O4, and O5. Corrective Action: Effective 05/29/2026, S1 stated she will create a file for the independent contractors and all new contractors before they provide services to children, to ensure compliance with this regulation.

Based on observation and record review at 1:15 p.m., S1 failed to have documentation of tornado drills that were conducted at least once per month during various times of the day in the months of March, April, and May. S1 failed to have tornado drills for the months of March and April 2026. Corrective Action: Effective 05/29/2026, S1 stated she will put a calendar reminder for the months of March, April, May, and June to complete tornado drills to ensure compliance with this regulation.

The Specialist reviewed camera footage from the center dated 04/08/2025, at 9:50 a.m., C1 was observed standing by the restroom door as another child walked into the restroom. The door to the restroom then closed while C1's finger was in the hinge of the door. The door opened up 3 seconds later, and a child walks out of the restroom. S3 is observed coming out of the restroom 8 seconds…

Based on observation at 10 a.m., on 04/10/2025, S1 failed to have microwave ovens in areas inaccessible to the children. The Specialist observed microwave ovens on low shelves in 3 different classrooms, the one-year-old classroom, the two and three-year-old classroom, and the three and five-year-old classroom, that the children could access. S1 removed the microwaves prior to the Specialist's departure. Corrective Action: Effective 04/10/2025, S1 stated she will ensure no microwave ovens are placed in the classrooms accessible to the children to ensure compliance with this regulation.

Based on observation at 10 a.m., on 04/10/2025, S1 failed to have microwave ovens in areas inaccessible to the children.
