Based on record review and interviews on 08/01/2025, at 11:30 a.m., S1 failed to notify the Department and DCFS within 24 hours of the following critical incident: On the morning of 07/24/2025, O1 reported to S1 that on 07/23/2025 C1, 3-years-old, told her S7 placed both hands around her neck and choked her because she was trying to go to the playground to retrieve her bow. Based on interviews and record review, although the center has implemented a Behavior Management Policy prohibiting physical punishment, S11 (DOH 7/22/2018) used physical punishment on C1, 3 years old, on 07/18/2024 at approximately 11:30 a.m. S3 and S5 observed S11 take off her shoe during lunch time and pop C1 on his hand to discipline him. S11 was terminated 07/18/2024.
