      1711.A.&B&D.&E.: Based on observations at 10:08 a.m. and 10:15 a.m., S11 and S19 failed to ensure the child to staff ratio was met at all times as Specialist observed the following: -S11 was supervising 8 children, ages 1-years-old. Two staff were needed to meet ratio. This t 10:09 a.m. when S6 entered the classroom. -S19 was supervising 12 children, ages 2 to 3-years-old. Two staff were needed to meet ratio. This was corrected at 10:16 a.m. when one child was transferred to S17’s classroom. S19 was left with a mixed ratio of 11:1. Corrective Action: Effective 1/7/2026, S1 stated she will…

      Based on record review and interview at 10:45 a.m., S22 failed to ensure bottled formula/breast milk for infants was labeled with the names of C1, 8-months-old and C2, 10-months old. On 12/16/2025, at 2:23 p.m., C1 was fed C2's breast milk bottle. This could not be corrected prior to the Specialist departure. Corrective Action: Effective 1/7/2026, S1 stated she has ordered bottle labels and parents are writing the names on the bottles before bringing them in. All teachers and managers are checking bottles randomly for names to ensure compliance with this regulation.
