C.: Based on record review and interviews at 11:30 a.m., S1 failed to notify the Department and Child Welfare within 24 hours of being made aware of the following critical incident: On 6/17/2025, at 8:46 a.m., O1 informed S1 that C1, 4-years-old, told her on the evening of 6/16/2025, that S3 dropped him. Per the center's video footage dated 6/16/2025, C1 was observed holding his back after S3 forcefully sat him down in a chair. Corrective Action: Effective 7/9/2025, S1 stated she will report all criticals within 24 hours of being made of aware to ensure compliance with this regulation.

Based on record review and interviews at 11:30 a.m., although the center has a behavior management policy in place, S3 (DOH 8/6/2020) was observed on camera using a prohibited method of discipline on 6/16/2025 at 5:36 p.m. as she grabbed C1, 4-years-old, by his arms and sat him down forcefully. S3 was suspended effective 6/18/2025. Corrective Action: Effective 7/7/2025, S1 stated all staff will have a training on 7/23/2025 to review proper behavior management, supervision, and mandated reporter training to ensure compliance with this regulation.

Based on observations at 11:30 a.m., S1 failed to ensure 2 of 5 vehicles had a current safety inspection. Inspection stickers expired on 04/2025 for both vehicles; License Plate N456959 Dec 26 and License Plate M22470 Dec 25. S1 stated the center isn't currently using those two vehicles. Corrective Action: Effective 7/7/2025, S1 stated she will have the vehicles inspected on 7/8/2025 and will set a reminder to get them inspected prior to the inspection expiring to ensure compliance with this regulation.

Based on record review and interview at 10:00 a.m., S1 failed to ensure the center's visitor's daily log was maintained to include first and last name, date of visit, arrival or departure times, and purpose of the visit as evidenced by the log dated 3/27 - 4/7/2025 was missing arrival and departure times for 4 of the 10 entries. This was corrected prior to the Specialist's departure. Corrective Action: Effective 4/9/2025, S1 stated she will remind staff to remind visitors to sign in/out and will check it at the end of the day to ensure compliance with this regulation.

Based on record review and interview at 10:00 a.m., S7 was observed on camera using improper lifting techniques when she lifted C1, 1 year-old, on 4/1/2025 at 10:32 a.m. forcefully by his arms and sat him down on the floor. S7 was terminated effective 4/2/2025. Corrective Action: Effective 4/9/2025, S1 stated all staff will complete a training on proper lifting and she will model to all staff the proper way to lift children to ensure compliance with this regulation.
