Based on record review and interview at 10:00 a.m., S2 failed to ensure the daily attendance log for children included the departure time of each child and the name of the person to whom the child was released. C1, was not signed out on 8 of 14 days and there is no signature for the person to whom the child was released for the following dates: March 3 -March 5, 2025, March 12, 2025, March 18-March 20, 2025, and April 1, 2025. Based on record review and interview at 10:00 a.m., S2 failed to ensure daily attendance records were maintained for Staff and Owners, to include the time of arrival and departure. S2 was signed in on 4/1/2025, 4/7/2025, 4/15/2025, and 4/22/2025, but failed to sign out. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 4/24/2025, S2 stated she will make sure she signs out daily to ensure compliance with this regulation.

Based on record review/interview at 10:00 a.m., S2 failed to have a qualified director or qualified director designee approved by the Department on-site, full-time during the day time hours of operation. S2, Director, was present 19.5 hours during the week of 3/31/2025-4/4/2025 and 19.5 hours during the week of 4/7/2025-4/11/2025. Corrective Action: Effective 4/28/2025, S2 stated she will keep documentation of her attendance at the center and available upon request to ensure compliance with this regulation.

1903.D.5. Based on observations at 10:00 a.m., S2 failed to ensure the number of children using a room did not exceed the 35 square feet per child requirement. The infant room 1 can accommodate 8 children, but 10 children, aged 6 months to 1 year-old, were present. This was corrected prior to the Specialist's departure. Corrective Action: Effective 4/24/2025, S2 stated she will move children to ensure room capacity is not exceeded and to ensure compliance with this regulation.

C. Based on interviews at 1:00 p.m., S1 failed to notify the Department within 24 hours and the parent immediately of the following critical incident: On 2/24/2025, at 5:26 p.m., O1, C1s Mother, was notified that he had a scratch on his face and they are not aware how it happened. 2/25/2025, at 5:30 p.m., S9 was asked by O2, C1s Father, how did he get the scratch on his face, he was advised the scratch happened at home. She stated she was not aware she needed to submit written notice to the Department. This could not be corrected prior to the Specialists departure.

Based on record review/interview at 1:00 p.m., S9 failed to have written authorization signed and dated by the parent to secure emergency medical treatment for C1, 2-years-old. This could not be completed prior to the Specialist departure.

Based on record review/interview at 1:00 p.m., S9 failed to have a qualified director or qualified director designee approved by the Department on the premises from 2/3/2025-2/7/2025, 2/10/2025-2/14/2025, 2/17/2025-2/21/2025, 2/24/2025-2/28/2025, 3/3/2025-3/7/2025, 3/10/2025-3/14/2025, 3/17/2025-3/21/2025 and 3/24/2025-3/28/2025. This could not be corrected prior to the Specialist departure.

Based on record review at 11:45 a.m., S1 failed to have documentation that S4 and S8 completed the DCFS online mandated reporter training within 7 days of the first day present at the center. -S4's hire date and first day present at the center was 2/18/2025.
