Based on record review and interview at 10:00 a.m., S2 failed to ensure daily attendance records were maintained for Staff and Owners, to include the time of arrival and departure. Based on interviews at 1:00 p.m., S1 failed to notify the Department within 24 hours and the parent immediately of the following critical incident: On 2/24/2025, at 5:26 p.m., O1, C1s Mother, was notified that he had a scratch on his face and they are not aware how it happened. 2/25/2025, at 5:30 p.m., S9 was asked by O2, C1s Father, how did he get the scratch on his face, he was advised the scratch happened at home. She stated she was not aware she needed to submit written notice to the Department. This could not be corrected prior to the Specialists departure.
