Based on record review at approximately 1:40 p.m., S1 failed to provide documentation that S3 (DOH:2/11/2026) had current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. On 5/28/2026, the Specialists observed S3 solely responsible for six children ,5 to 8-year-olds during rest time. S1 corrected this by combining S3's class with S6 and S7's class, while still meeting child- to staff ratio requirements. 1903.C. Based on observation at approximately 8:50 a.m., S1 failed to ensure the outdoor area was free of hazards. The Specialist observed a hole in the front plastic window panel at the second building. The hole has sharp edges exposed. This window is accessible to children from the outdoor play area. S1 corrected this by sealing the hole with tape. Corrective Action: Effective 4/29/2026, S1 stated she will let the owner of the building know that the window needs to be repaired. She will do visual checks of the outdoor area to ensure no hazards are present daily.

Based on record review at approximately 9:30 a.m., S1 lacked documentation of a current approval from Office of Public Health. The date of the last approval expired 04/08/2026. This could not be corrected prior to the Specialist departure. Corrective Action: Effective 4/29/2026, S1 stated she will contact the Office of Public Health to schedule a visit. She will also keep a calendar of inspections expiration dates to ensure compliance with this regulation.

Based on record review/interview at 2:30 p.m., S1 failed to ensure the daily attendance records for visitors accurately reflected the visitors on the center premises at any given time. On 11/13/2025, S1 failed to have Bossier City Police Officers sign in and out on the visitor’s log. This was not corrected prior to the Specialist departure. Corrective Action: Effective 12/1/2025, S1 stated she will make sure all visitor’s sign in and out to ensure compliance with this regulation.

1509.A.8.a.&b.: Based on record review/interview at 2:30 p.m., S5 (DOH 9/8/2025) used a prohibited method of discipline on 11/12/2025 as observed by S1 via video footage. S1 stated on 11/12/2025, at approximately 6:30 p.m., she reviewed the camera footage and observed the following:-At 7:56:33 a.m., S5 grabbed C1, 4-years-old, by his right arm and pushed him down onto the blue bean chair located in the corner of the classroom.-At 7:56:57 a.m., S5 grabbed C1 again by his collar and the hood of his coat and pushed him face down onto the blue bean bag chair.S5 was immediately…

Based on observations at 1:30 p.m., S1 failed to have a current vehicle safety inspection sticker. The current inspection sticker expired 3/2024. This was not corrected prior to the Specialist departure. Corrective Action: Effective 12/1/2025, S1 stated she will check at the beginning of the month for expiration dates to ensure compliance with this regulation.
